Dr. Sara Meltzer (Traditional Medicine)
Dr. Meltzer graduated from the University of Alberta Medical School. She went to McGill for specialty
training in Internal Medicine and Endocrinology, with 1 year spent in Endocrinology at the University
of Paris, France. She is a staff endocrinologist at the McGill University Health Centre - Royal Victoria
Hospital and Associate Professor at McGill University in Medicine and Obstetrics and Gynecology.
Dr. Meltzer has been actively involved in Diabetes Quebec and the Canadian Diabetes Association
in the development of educational materials and was chair of the steering committee for the 1998
Guidelines for the Management of Diabetes in Canada. For her participation in the 1998 guideline
development team, she received the Banting Award in 1999 from the CDA. In 2001, she was
awarded the Wong Award, also from the CDA, for her “outstanding service and contribution for
people with diabetes”. For the 2003 revision of the Canadian Guidelines for the Prevention and
Management of Diabetes in Canada, she was subcommittee chair for pregnancy and a member of
the Steering Committee. She worked with Health Canada “Care” Working Group to develop a “blueprint” for a National Diabetes Strategy for 2004-2009.

Dr. Ernest McCrank (Complementary Medicine/Physicians’ Health)
Ernie McCrank is a Professor Emeritus in the Department of Psychiatry at the University of Western
Ontario. He is presently in private practice in Calgary, Alberta and sits as a member of the Research
and Ethics Committee of the College of Physicians and Surgeons of Alberta. Dr. McCrank has been
on the Teaching Faculty in the Department of Psychiatry at Queen’s University, the University of
Ottawa, and the University of Western Ontario. He was Chief of Psychiatry and Chairman of the
Medical Advisory Committee at University Hospital in London, Ontario and has trained in Internal
Medicine at the Cleveland Clinic. He is primarily interested in the spectrum of anxiety disorders, stress
disorders and mind-body interaction in physical disease and has been a juvenile diabetic for over 50
years.

Dr. Robert D. Horvath (Practice Management)
Dr. Horvath, a graduate of the University of Toronto, has practiced Family and Emergency Medicine
since 1982. He now works as a full time emergency room physician at both divisions of North York
General Hospital in Toronto, Ontario. An Assistant Professor in the Department of Family and
Community Medicine of the University of Toronto, Dr. Horvath teaches students in Family Medicine
and Emergency Medicine. Dr. Horvath is the CEO of Rosterware Inc., which produces state-of-the-art
Internet-based scheduling applications for Health Care. He has given several lectures and
presentations on electronic medical records and electronic information exchange. |

23. He is a Certified Financial Planner, Chartered Life Underwriter
and Chartered Financial Consultant and has spent the past 15
years specializing in retirement, tax and estate planning. Greg
has been instrumental in reshaping how numerous physician groups
are compensated, with a focus on improving their current and
future lifestyle. Some of the groups he has worked with include
Neo-Natologists, Radiologists, Thoracic Surgeons, Neurologists
and the
